#2 - Fill out the New Patient paperwork prior to arriving to expedite you appointment.

#3 - Arrange a ride to and from your appointment
During your appointment, you eyes may be dilated, numbed, or given an injection depending on your specific treatment plan.
Many times, our patients leave with blurred vision before their dilation medication wears off.
Better to be safe than sorry!

Dr. Sunil M.Malkani Welcomes You to Our Family
Dr. Malkani and his staff share a common mission to provide the most up-to-date medical and surgical care in an environment that is compassionate and patient focused. Upon arrival at the office you will be greeted by our warm friendly staff. Please remember to bring a photo ID, your insurance card (s) and a list of all the medications you are currently taking. Your eyes will be dilated for your examination which may result in blurred vision and sensitivity to light for several hours; it is recommended that you have someone drive you to and from your appointment.
